OUR MISSION
Bridge Care Connections is a nonprofit organization dedicated to advancing health equity by
providing healthcare education, care management, and community-based support to
underserved and minority populations. Our work focuses on reducing disparities and connecting
communities to the care they deserve.
Many individuals face significant challenges navigating complex healthcare systems.
Low-income families, Medicaid recipients, minority populations, and individuals experiencing
transitional life circumstances often struggle to access the medical and mental health services
they need due to lack of knowledge, limited support systems, and systemic barriers.
Bridge Care Connections was founded to address these challenges by providing accessible
education, resource navigation, and advocacy support that empower individuals to better
understand healthcare systems and connect with the services available to them.
The organization’s first program, the Health Navigation Education Program, will launch in 2026
and provide individuals with the tools and knowledge needed to effectively navigate healthcare
systems, understand insurance and Medicaid benefits, access mental health and medical
providers, and advocate for themselves within healthcare settings.
This program will be delivered through online educational modules, downloadable learning
resources, community outreach efforts, and a developing healthcare resource directory
designed to connect individuals to appropriate services.
Our goal is to resolve over 25,000 barriers to care—connecting individuals and communities to the support they deserve.

At Bridge Care Connections, we understand that barriers don’t always come in obvious forms. Sometimes it’s financial. Sometimes it’s timing, access, or simply not knowing where to start.

We created our Access Scholarship Program because we’ve seen firsthand how one barrier can prevent someone from receiving the support they need.

While our resources are limited, our intention is not. We are committed to reducing barriers where we can—whether that means providing a free resource navigation session for an individual or offering discounted training to small organizations working directly within their communities.

Every request is reviewed with care, because we recognize that not every situation fits into a predefined category. Our goal is not just to provide services, but to create access in a way that is thoughtful, realistic, and impactful.
