For many individuals—especially those navigating new or complex systems—appointments can feel overwhelming, rushed, and confusing.
Whether it’s a medical visit, a housing intake, or a case management meeting, people are often expected to advocate for themselves without ever being shown how.
At Bridge Care Connections, we believe access to services is only part of the solution.
People also need the tools to navigate those services effectively.
That’s why we created the BRIDGE Method.
What is the BRIDGE Method?
The BRIDGE Method is a simple, structured approach designed to help individuals feel more prepared, confident, and in control during appointments.
It focuses on six key actions:
B — Bring Your Info
Come prepared with important documents, medications, and a list of concerns or symptoms. Preparation sets the tone for a more productive appointment.
R — Raise Your Questions
Asking questions is not being difficult—it’s being informed. If something is unclear, ask for clarification until you understand.
I — Include Yourself
You are not just receiving services—you are part of the decision-making process. Your voice matters in every step.
D — Don’t Leave Confused
Before leaving, make sure you understand your next steps, instructions, and any follow-up needed.
G — Get Support
If possible, bring a trusted person or ask for additional support. You don’t have to navigate important decisions alone.
E — Escalate Concerns
If something feels wrong or you are not being heard, you have the right to speak up, request a supervisor, or seek additional support.
Why This Matters
Many individuals—especially those transitioning into independence or accessing services for the first time—experience:
Confusion about next steps
Lack of clear communication
Feeling dismissed or unheard
Missed opportunities for better care or support
The BRIDGE Method helps close that gap by providing a clear, repeatable structure.
How We Use It
At Bridge Care Connections, the BRIDGE Method is integrated into our case management approach.
Participants receive:
A personalized appointment summary
Pre-filled questions tailored to their needs
A take-home version of the BRIDGE Method
We also provide simplified versions of this tool to the broader community—because everyone deserves access to this kind of guidance.
Final Thought
Advocacy shouldn’t be something people have to figure out on their own.
With the right tools, preparation, and support, every individual can walk into an appointment with more confidence—and walk out with clarity.
Your care. Your voice. Your right.
