One of our Founders greatest inspirations is Dr. Sergio Aguilar‑Gaxiola, the founding director of the Center for Reducing Health Disparities at UC Davis Health in Sacramento, California.

Dr. Aguilar‑Gaxiola doesn’t just study health inequities — he lives the mission. When he talks about caring for communities, he says there was never a start to his passion for service. It wasn’t something he chose — it’s in his DNA, just like it’s in the team of Bridge Care Connections .

His center works every day to break down barriers that keep people from getting the care they deserve. They pursue research, outreach, training, and education that improve access, early intervention, and culturally‑appropriate care for underserved communities — especially for people whose voices have too often been ignored by the traditional system.

The Center for Reducing Health Disparities is located at:

📍 2921 Stockton Blvd., Suite 1400, Sacramento, CA 95817 (UC Davis Clinical & Translational Science Center).

They help build trust with communities through outreach, interpretation services, telehealth connections, and by working with people where they live, work, and gather — not just waiting for them to come to services.

Dr. Aguilar‑Gaxiola’s example reminds is why we care beyond the surface. That’s why I’m placing the Center for Reducing Health Disparities at the top of our directory — acknowledging all they do to meet people where they are, empower communities, and make health care more equitable.

Thank you, Dr. Aguilar‑Gaxiola, for your leadership, your heart, and your lifelong commitment to people who too often go unheard.

TOGETHER WE BRIDGE
